Description: The ACT on Health Equity Community Solutions Challenge is designed to support non-profit organizations positively impacting the health of underserved populations through regional and local community-based programming. Applications will be accepted in two categories: 1) community health and wellbeing [community-based programs to improve conditions that affect health and well-being (e.g., housing, education, environmental/neighborhood safety, nutrition, access to care) and programs that prioritize the social, cultural, and linguistic needs of underserved patients, caregivers and families]; and 2) youth STEM education and career readiness (programs to build knowledge and skills in the fields of science, technology, engineering and math among K-12 or undergraduate students.
Award: Up to $25,000 over a one-year period.
Eligibility: Open to all eligible 501(c)(3) organizations in the U.S. delivering regional and local community-based programming, including local chapters of national organizations.
